Child Hosting Forms
If you wish to apply to host a child, please download the following forms. The full instructions for the required Police Checks are in the Potential Host Family Letter. Please read it carefully as Police checks are different depending upon where you live. All forms are required for applying to host a child. Membership Form
This form is to apply to become a member of Canadian Aid for Chernobyl or to renew your annual membership.
Education & Scholarship Application
This form is used by members sponsoring a child’s education in Belarus. It must be completed, submitted and approved by the committee for a tax receipt.
Humanitarian Assistance Application
This form is used by members for humanitarian assistance in Belarus. It must be completed, submitted and approved by the committee for a tax receipt.

CAC Aeroplan Miles Pledge Form
This form is used to donate Air Canada Aeroplan Miles to CAC’s Aeroplan Pooling Account. This drive is only valid for one month of each calendar year and the 2011 drive will be held from November 1 – 30. Any number of miles can be donated and these miles will be used either to bring someone for medical care in Canada or to take someone to Belarus for a Humanitarian Aid Mission.
